Update (2/22/2016):

The Early Registration deadline has been extended! Early Registration must be received/postmarked by 11:59 pm CT on Monday, March 14, 2016. Please note that this is the only extension that will be offered.

Registration forms received after Monday, April 11, 2016 will not be processed. Please register online or onsite if you do not meet the April 11 deadline. Online registration will be available up to and during the meeting. Onsite registration will also be available during the hours listed below.

If you are not currently a member and wish to join to receive the discounted registration rate, please go to the American Cochlear Implant Alliance website at www.acialliance.org and click on the Join Us tab. Professionals who are members either through their Organization or as an Individual Professional Member may receive the discount. Please note that Consumer Members are not eligible for the discounted registration.

Confirmations:

Formal confirmations will be sent to all attendees within 10 business days of receipt.

Substitution and Cancellation Policy:

Substitutions: Substitution of one individual to another is not permitted.
Cancellation: Cancellation requests must be made in writing and received on or before Monday, April 11, 2016. There is a $50 handling fee for all refunds and returned checks. Cancellations and registrations postmarked after the deadline date will not be eligible for refunds.
